چکیده
I t was good to see the interesting article by Bergenstal et al. (1), from both academic and commercial colleagues, of a novel approach to a long-acting insulin (LY2605541), possibly hepatospecific, as studied in individuals with type 2 diabetes. Both alanine aminotransferase (ALT) and serum triglycerides were higher compared with a standard insulin comparator, whereas body weight trajectory was lower. This raises important questions for further insulin development. It would be unusual to measure serum triglycerides and not other serum lipids in such a study. Indeed it would be unusual not to make total, LDL, and HDL cholesterol measurements in a phase 2 study of individuals with any type of diabetes. Given the ALT and triglycerides differences, one might expect lower serum HDL cholesterol. Such lipid profile results are not given in the article, nor highsensitivity C-reactive protein, which might also have deteriorated with the ALT. With the ALT change, which might indicate increased liver fat, it could also be speculated that insulin insensitivity in the fasting state would deteriorate, but insulin/C-peptide results are not given, nor a derivative measure such as homeostasis model assessment of insulin sensitivy (HOMA%S). This commentator would go further and expect differences in plasma free fatty acids, and eventually, though perhaps not in 12 weeks, differences in islet b-cell function, as homeostasis model assessment of b-cell function (HOMA%B). Are any such data available?

We thank Professor Home for his interest (1) in our publication on a novel basal insulin analog, LY2605541 (2). Total, LDL, and HDL cholesterol levels along with triglyceride levels (or lipid profiles) were measured as noted in the last paragraph on page 2144 (2). Whereas the serum triglyceride measurements for the LY2605541-treated patients did not differ from baseline but were higher compared...
متن کاملBetter Glycemic Control and Weight Loss With the Novel Long-Acting Basal Insulin LY2605541 Compared With Insulin Glargine in Type 1 Diabetes
OBJECTIVE To compare effects of LY2605541 versus insulin glargine on daily mean blood glucose as part of a basal-bolus regimen for type 1 diabetes. RESEARCH DESIGN AND METHODS In this randomized, Phase 2, open-label, 2 × 2 crossover study, 137 patients received once-daily basal insulin (LY2605541 or glargine) plus mealtime insulin for 8 weeks, followed by crossover treatment for 8 weeks. Daily...

OBJECTIVE To evaluate whether LY2605541 results in lower fasting blood glucose (FBG) versus insulin glargine (GL). RESEARCH DESIGN AND METHODS This 12-week, randomized, open-label, Phase 2 study enrolled patients with type 2 diabetes (hemoglobin A(1c) [A1C] ≤ 10.5%), taking metformin and/or sulfonylurea with GL or NPH insulin once daily. Patients converted to morning insulin administration du...
متن کاملLower glucose variability and hypoglycemia measured by continuous glucose monitoring with novel long-acting insulin LY2605541 versus insulin glargine.
OBJECTIVE To use continuous glucose monitoring (CGM) to evaluate the impact of the novel, long-acting basal insulin analog LY2605541 on hypoglycemia and glycemic variability in patients with type 2 diabetes. RESEARCH DESIGN AND METHODS Hypoglycemia and glucose variability were assessed with CGM of interstitial glucose (IG) in a subset of patients with type 2 diabetes from a phase II, randomiz...

OBJECTIVE To determine the safety and efficacy of the long-acting insulin analog, insulin glargine, as a component of basal bolus therapy in patients with type 1 diabetes. RESEARCH DESIGN AND METHODS Patients with type 1 diabetes receiving basal-bolus insulin treatment with NPH human insulin and insulin lispro were randomized to receive insulin glargine (HOE 901), a long-acting basal insulin ...
